Rhabdomyolysis: Spotting the Danger Before It’s Too Late

Rhabdomyolysis, commonly referred to as "rhabdo," is a condition that every fitness enthusiast should be aware of. It occurs when muscle tissue breaks down rapidly, releasing proteins like myoglobin into the bloodstream. This can overwhelm the kidneys and lead to serious complications, including kidney damage. Though rare, rhabdo can affect anyone—from elite athletes to those engaging in a seemingly normal workout.

The symptoms of rhabdo can be deceptive. Muscle soreness and fatigue are common after exercise, but extreme pain, weakness, swelling, and dark-colored urine are warning signs that should never be ignored.  

This last week I worked with a client who works out on a regular basis for health and wellness.  After a new workout routine with more calf raises than normal, she experienced severe muscle cramping and thankfully sent a text message to her doctor.  Her doctor told her to get to urgent care and get a blood test run.  My client needed IV fluids to recover after what she thought was a routine workout. Her case highlights how quickly rhabdo can escalate, especially when hydration or recovery protocols are overlooked.

Preventing rhabdo begins with knowing your limits and easing into new or intense workouts. Gradual progression is key to allowing your muscles to adapt without undue strain. Incorporating hands-on sports therapy as you are recovering from a case such as this can be highly beneficial.  We will want to wait until the severity of the symptoms has passed and you have the OK from your doctor before we begin therapy.  When we do begin, there won’t be any deep tissue work, only recovery work.  Massage can reduce some of the muscle tightness and support recovery. Regular therapy sessions not only alleviate soreness but also help maintain muscle health, especially during rigorous training periods.

Hydration is crucial for prevention and recovery. Dehydration increases the risk of rhabdo, so staying properly hydrated before, during, and after workouts is essential. Nutritional choices play a significant role, too. A balanced diet with lean proteins, complex carbohydrates, and healthy fats provides the nutrients your body needs to repair and grow stronger. While rhabdo is a serious condition, it’s manageable with early intervention.
